Employee Exit Process

(for voluntary resignations)

In support of Boise State University’s strategic plan to attract, retain, reward and develop a high-quality workforce, Human Resource Services (HRS) has developed the following exit processes.  By partnering with departments to identify those employees who have given their notice of resignation, we can ensure prompt notification of and contact with exiting employees.  This website includes specific instructions for the Employee Exit Instructions and the Supervisor’s Exit Instructions.

It is critical that employees who are leaving the University receive timely information relative to the University’s exit process to ensure they have returned all university property, cleared all acquired debts, received important information regarding their benefit options, and updated their contact and reference preferences.

Employee Exit Instructions

(for voluntary resignations)

Below is the process you will need to follow to end your employment relationship with Boise State University.  This will ensure:  1) Your separation is processed as efficiently as possible.  2) That you are informed of important facts/information affecting your benefits.  3) That all university property is returned and that all personal debts are cleared with the University prior to your departure.  We recommend this is done with as much lead time as possible.

If you have any questions about this process, please contact HRS at 426-1616. 

  1. Complete an Employee Voluntary Resignation Notification Form pdf | doc and submit it to your immediate supervisor who will sign it and return it to HRS.
  2. When HRS receives completed Employee Voluntary Resignation Notification Form, an email confirmation will be sent to you with important benefit information (if applicable).
  3. Complete the Separation Checklist pdf | doc document (per instructions on the form) and submit it to your supervisor for review and acknowledgement.  Your supervisor will send the completed checklist to HRS for processing and documentation.
  4. Complete the Reference Authorization Release Form pdf | doc and send original to HRS and copy to your supervisor.  If this form is not completed and sent to HRS, only name, business title and dates of employment will be released.

Supervisor’s Exit Instructions

(for voluntary resignations)

Below is the exit process you and your employee will need to follow to ensure:  1) The resignation is processed as efficiently as possible, 2) The employee is informed of important facts and information affecting benefit options, and 3) That all accounts are cleared and University property is returned prior to resignation.  If you or your employee have any questions about this process, please contact HRS at 426-1616. 

  1. Refer the departing employee to the on-line Employee Voluntary Resignation Notification Form pdf | doc to complete and return to you (supervisor).
  2. Accept employee’s resignation by signing the resignation notification form and return it to HRS.  (You may wish to discuss this with your management prior to sending to HRS).
  3. When HRS receives completed Employee Voluntary Resignation Notification Form pdf | doc, an email confirmation will be sent to the employee with important benefit and other information
  4. If this is not a voluntary resignation, you should be working with an HRS Employee Relations representative on the appropriate exit process. 
  5. Ask your employee to complete a Separation Checklist pdf | doc and return it to you for review and signature.  After the form is completed and signed by you, return it to HRS.
  6. If you receive a call for reference request information, call HRS to confirm what information the employee has authorized the University to release.
  7. The department is responsible for initiating an EAF for professional staff and faculty employees.  HRS prepares EAF for classified staff.
