Eligibility

An eligible Member who is in the employ of the State of Idaho; and the State of Idaho has the reasonable expectation that such employment will exceed five months in any 12-month period and twenty hours per week, or 84 hours per month, during the five-month period. The effective date is the first day of the month following date of employment.


Enrollment

Employees must complete an application with Human Resource Services (HRS).


Summary of Benefits

Basic Life Insurance pays the beneficiary 100% of annual salary, but not less than $20,000 for the employee. For the employee’s spouse, the amount is $2,000. For the employee’s children, who are more than 10 days old, but less than termination age, is $1,000. The plan includes an Accidental Death & Dismemberment (AD&D) provision. AD&D benefits are in addition to any paid by Basic Life benefits or other State life insurance plans.

Supplemental Life Insurance pays the beneficiary 100% of annual salary.


Premiums

Premiums for the Basic Life Insurance are paid by Boise State University.

Premiums for Supplemental Life Insurance are paid by the employee based on a rate per their age and annual income. Example: Annual Salary = $32,750, Age = 37, $0.12 x 33 = $3.96 monthly premium
